Reporting

All accidents or incidents including near misses, injuries and hazards must be reported to the relevant West Gippsland Catchment Management Authority (WGCMA) project coordinator.

A WGCMA incident report, available from WGCMA project coordinators, must be completed as soon as possible for all accidents/incidents including near misses, injuries and hazards.

You will notify WorkSafe Victoria on the emergency number, 132 360 for all notifiable incidents and follow the correct incident reporting guidelines issued by WorkSafe. You will ensure the site is not disturbed until an inspector arrives or directs otherwise.

Notifiable incidents include:

  • death
  • employees or person who require either:
    – medical treatment by a doctor (for example, fractures, administration of a drug or medical treatment)
    – immediate treatment as an in-patient in a hospital
  • incidents involving high risk equipment or plant listed in the  Equipment (Public Safety) Regulations 2017

The person nominated to notify WorkSafe Victoria must send a written record of the incident to the Victorian WorkCover Authority within 48 hours by going to www.worksafe.vic.gov.au and completing an online incident notification form.
